Important Appointment Message

Your appointment is extremely important to us.  Each appointment is allotted a specific amount of time to attend to your needs in a timely manner.  We encourage all patients to arrive fifteen minutes prior to their scheduled appointment to allow time for all necessary forms to be filled out so that your appointment can begin on time.  To save time you can go to our new patient forms section, print and fill out the forms, and bring them with you to your appointment.

If you cannot make your scheduled appointment or would like to change your appointment please contact the front desk at least 24-hours in advance.  This enables us to fill the allotted time slot for another patient.  In order to maintain a professional standard of care we do have a cancellation and missed appointment policy. A fee of $50 (Well Women) or $25  (for other types of visits) may apply for no shows or cancellations with less than 24-hours notice.

$45 Fee for prior authorization. Some prescriptions require Prior Authorization before they will be covered by your insurer.
